  • Job Objective: A brief overview of the position.
    • This position is responsible for providing personal care and mental and physical stimulation to the adult day program clients in a safe and "failure free" environment.

    • Education
      • N/A
    • Licensure/Certification
      • Required:American Heart Association CPR and First Aid Certification; finger print clearance (may be obtained within 30 days of hire)
    • Experience
      • Preferred: Six (6) months providing care to frail elderly.
  • Essential Responsibilities
    • Demonstrates compliance with Code of Conduct and compliance policies, and takes action to resolve compliance questions or concerns and report suspected violations.
    • Performs personal care procedures according to protocol.
    • Responsive to client’s needs so that quality client care is provided and individualized program plans of care are implemented.
    • Leads activity groups so that clients respond with interest and participate at their highest functional level.
    • Demonstrates ability to plan and implement assigned activity groups and personal care to clients as scheduled on the daily activity plan
    • Records timely and accurate client care and services as assigned on the schedule due date.
    • Changes assignments or take on additional assignments as the workload and client needs require.
    • Provide a "Failure Free" Environment.
    • Maintain thorough and timely continence schedules.
    • Prepares for the serving of the noon meal, takes temperature of food and documents, serves and cleans up everyone has eaten.
    • Plans, prepares, serves, cleans up after and documents AM and PM snack.
    • Uses free time between assignments to complete the following: maintain a safe, clean work environment, ensure that department supplies are adequate, follow through with special projects as assigned and maintain current documentation in client records.
    • Performs other duties as assigned.
