  • Job Objective: A brief overview of the position.
    • This position is responsible for the planning, organizing and directing of the daily operation of the memory care adult day care to improve the quality of life for families living with cognitive impairment.

  • Qualifications
    • Education
      • Required: Bachelor Degree in the human services field 
    • Licensure/Certification
      • Required: Fingerprint clearance, American Heart Association BLS and First Aid Certification
    • Experience
      • Required: Two years professional service experience with adults in cognitive decline
      • Required: 2 years in management or supervision role
      • Preferred: Experience in adult day services setting, EPIC billing system, bookkeeping
  • Essential Responsibilities
    • Demonstrates compliance with Code of Conduct and compliance policies, and takes action to resolve compliance questions or concerns and report suspected violations
    • Performs client/family assessments
    • Performs ongoing case management and counseling for client/family
    • On boards new families
    • Facilitates support groups and community education programs
    • Coordinates, evaluates and ensures quality client care for cognitively/functionally impaired adults
    • Monitor TB and Flu testing
    • Manages staff including: coaching, counseling, hiring and training
    • Develops policies and procedures
    • Plans and implements process improvement strategies
    • Leads case management meetings and ensures individual assessments and program plans for clients
    • Coordinates and provides documentation of department staff meetings and in-services
    • Works closely with finance to calculates and report department statistics
    • Tracks spending on government grants and reports on monthly basis to government agencies
    • Coordinates billing of client accounts
    • Fulfils public speaker engagements for community education
    • Evaluates and ensures compliance with licensing/grant requirements
    • Participates in the preparation and monitoring of budget to accomplish fiscal objectives of the department
    • Implements cost containment measures
    • Maintains consistency within the centers daily routine
    • Recruits and trains volunteers
    • Completes special projects as assigned
