Job Description:

Education:

Required: Bachelor’s degree in Nursing or Master’s degree in Nursing

Licensure/Certification:

Required: California RN licensure and Orthopedic Nurses Certification within one year of hire

Experience:

Required: Five (5) years in clinical setting working as an RN
Preferred: Leadership experience, quality improvement experience, orthopedic experience

Job Objective:

Plans, implements and directs activities to meet orthopedic patient and clinician needs to comply with accreditation standards. Integral to the role is patient care coordination, assessment and analysis of process and outcome data, education planning, multidisciplinary collaboration, and communication.

Essential Responsibilities:

  • Demonstrates compliance with Code of Conduct and compliance policies, and takes action to resolve compliance questions or concerns and report suspected violations.
  • Coordinates actions to meet TJC Orthopedic Disease Specific Certification standards.
  • Actively participates in internal and external committee meetings that support &/or impact the Orthopedic Programs and chairs Interprofessional Orthopedic Committees in collaboration with the Orthopedic and Spine Program Medical Directors.
  • Facilitates execution of changing clinical practice guidelines.
  • Develops and coordinates program performance improvement plan to meet strategic goals.
  • Analyzes performance data, identifies process gaps and interprets significance to stakeholder.
  • Recommends opportunity for improvement to meet performance standards.
  • Collaboratively leads improvement changes to meet target outcomes.
  • Coordinates clinical decision aides within the electronic medical record (EHR) in collaboration with clinical experts by aligning clinical practice guidelines and accreditation standards.
  • Oversees orthopedic patient care plan across the continuum through leadership and collaboration with department directors and their bedside clinicians.
  • Facilitates nurse navigation across the continuum in partnership with the nurse navigator, as needed, for orthopedic and spine patients.
  • Supports resource utilization considering factors related to safety, effectiveness, cost and impact on practice in the planning and deliver of orthopedic care.
  • Builds effective partnerships. Acts as liaison between stakeholders: Clinic, PAT, Preop/OR/PACU, Inpatient Orthopedic Unit, Orthopedic Physicians, Anesthesia, and Therapy.
  • Contributes to inpatient and outpatient orthopedic team meetings; recommends care and community resources as needed.
  • Supports education in self and others; pursues self-learning opportunities to gain new knowledge and improve skills.
  • Coordinates orthopedic education program to meet healthcare team and community education needs.
  • Supports Magnet Model; role models nursing leadership and professional standards promoting magnet model of nursing care.

Essential Skils:

  • Knowledge of Joint Commission Disease Specific Certification standards and performance measurement
  • Able to aggregate, analyze and present data using spreadsheets (e.g. Excel) or data base software
  • Adapts to rapidly changing environments with flexibility
  • Able to work autonomously and exercise high degree of initiative
  • Strong verbal, written and interpersonal communication skills
  • Knowledge of clinical course of care in an acute care setting
  • Knowledge and ability to function in a professional role and serve as a professional role model with the Orthopedic/Spine patient population
  • Knowledge of current nursing, medical and health trends and practices in orthopedics
