  • Essential Responsibilities
    • Demonstrates compliance with Code of Conduct and compliance policies, and takes action to resolve compliance questions or concerns and report suspected violations.
    • Performs assessments on acute stroke patients to establish and record incoming patient condition
    • With Stroke Service Team, initiates stroke orders and treatment plan to provide acute care, including on-going assessment, routine procedures, documentation in patient chart, family/patient counseling, education, and discharge planning.
    • Demonstrates implementation skills in accordance with EMH Standards of Care, policies, procedures, and Nurse Practice Act.
    • With the supervision of a Physician, serves as a primary responder with the Stroke Service Team for acute cerebrovascular patients entering the system.
    • Demonstrates ability to change intervention strategies when expected outcomes not met.
    • Consistently documents in accordance with EMH policies and procedures.
    • Assesses and analyzes outcome Measures for continued JCAHO Stroke Center certification.
    • Demonstrates competency in the operation of equipment specific to clinical area.
    • Provides accurate and precise reporting of patient condition and changes in a timely manner to physicians.
    • Identifies educational needs and provides comprehensive education pertaining to stroke intervention, prevention and rehabilitation for inpatients and community
    • Collaborates with nursing management, physicians and the Education Department to identify gaps in processes, develop and implement staff education related to stroke issues.
    • Completes age specific competency annually.
    • Coordinate research, collect and collate data and report results.
    • Performs daily rounding of stroke patients to coordinate care , treatment plan and nursing interventions
    • Complies with departmental plan to meet budget goals.
    • Demonstrates knowledge of Department of Nursing and Joint Commission required quality indicators, performance improvement and patient safety goals.
    • Develops and runs stroke clinic in coordination with ENI Director
