  • Job Objective: A brief overview of the position.
    • The Transfer Center Coordinator position ensures the day-to-day staffing needs of each unit are maintained. Works with the Transfer center RN in facilitating incoming and outgoing transfers. Coordinates additional activities centrally located in the transfer center and promotes and maintains a professional image through contact within and outside the organization.

      • Required: One (1) year of experience in healthcare, preferably in acute care or case management. 
  • Essential Responsibilities
    • Demonstrates compliance with Code of Conduct and compliance policies, and takes action to resolve compliance questions or concerns and report suspected violations.
    • Functions as a role model for organizational values.
    • Demonstrate positive written and verbal communication skills with physicians and staff. Exhibit mature behavior when directing and enhancing effective problem solving.
    • Supports Directors of the various units in a manner that helps to meet agreed upon benchmark standards in terms of Productivity, Efficiency and Financial Stability.
    • Processes Kronos entries and develops extensive knowledge of Kronos & its capabilities.
    • Inputs schedule changes in collaboration with each unit Director. Assists staff in finding a replacement when a schedule change needs to be made. Assists in monitoring compliance of Self-scheduling Guidelines and reports any inconsistencies to Directors of Nursing Resource Management.
    • Assesses shift staffing needs consistent with patient mix, census and acuity levels. Strives to maintain budgetary goals, making reassignments, calling replacement personnel and canceling personnel according to established procedures.
    • Partners with Travelers and Registry personnel to meet nurse/patient ratios with a pro-active strategy.
    • Enters daily shift changes and sick calls. Enters shift change census and midnight census. Maintains current telephone numbers of all Patient Care Services staff for use by management and staffing office personnel.
    • Ensures compliance with EMH contract labor contracts in regard to hours worked. Demonstrates knowledge of and initiative to ensure all records for registries are maintained and meet JACAHO requirements.
    • Verifies registry staff and traveler hours worked.
    • Generates attendance reports when requested by Directors.
    • Works in coordination with the Transfer Center RN to communicate with outside agencies regarding incoming and outgoing transfers.
    • Assists the transfer center RN in gathering sufficient and relevant information from all sources in compliance with regulating requirements and laws, to facilitate appropriate admission to EMC in an accurate, safe, timely, and cost effective manner.
    • Makes and follows up on transportation arrangements per the direction of the transfer center RN.
    • Develops and maintains bed search tracking method and utilizes to maintain documentation of patient placement status including transportation.
    • Promotes positive climate and morale of the hospital. Develops effective relationships with peers and staff; provides recognition for good work performance and gives positive feedback.
    • Implements and maintains Five Star Standards for patients and visitors. Monitors department specific standards of care practice.
    • Maintains high degree of confidentiality regarding termination of staff, staff personal excused absences and during times of coverage of the Nursing Administration Office.
    • Assists in monitoring hospital readiness in JACAHO, California and Federal standards.
    • Assists with collection of data for nursing P.I. projects.
    • Performs duties involved in the release of the deceased to the Mortuary.
    • Performs other duties as assigned.
