Job Description:

Education:

Required: Bachelor’s degree in Nursing or Master’s degree in Nursing or enrollment in an RN-Bachelor’s degree in Nursing or RN-Master’s degree in Nursing program within 1 year of hire and completion within 5 years of hire if hired after July 1, 2012
Preferred: Master’s degree in Nursing

Licensure/Certification:

Required: California RN licensure, American Heart Association BLS

Preferred: Certified in specialty practice area

Experience:

Required: Two (2) to Five (5) years in clinical setting Knowledge of current nursing, medical and health trends and practices in rendering of patient care Knowledge and ability to function in a professional role and serve as a professional role model
Preferred: Palliative care, chronic disease management, case management, utilization management or adult acute care experience

Job Objective:

Coordinates individualized assistance to patients, families, and caregivers who are seeking palliative care interventions and help overcome healthcare system barriers. As a professional registered nurse with specific clinical knowledg, uses the nursing process to provide education and resources which facilitate informed decision making while supporting quality health and psycho social care through all phases of the care continuum.

Essential Responsibilities:

  • Demonstrates compliance with Code of Conduct and compliance policies, and takes action to resolve compliance questions or concerns and report suspected violations.
  • Collects comprehensive data pertinent to the patient’s health or situation.
  • Analyzes the assessment data to determine the diagnoses or issues.
  • Identifies expected outcomes for a plan individualized to the patient or the situation in alignment with the palliative care team.
  • Collaborates to develop a plan of care strategies and alternatives to attain expected outcomes across the care continuum to support palliative care.
  • Implements the identified plan.
  • Evaluates progress toward attainment of outcomes and revise as needed to meet goals.
  • Systematically enhances the quality and effectiveness of nursing practice using the Relationship Based Care model and shared governance for decision making.
  • Attains knowledge and competency that reflects current evidence based practice for nursing care; facilitates health and disease patient education in the in-patient and out-patient care continuum.
  • Evaluates personal nursing practice in relation to professional practice standards and guidelines, relevant statutes, rules and regulations; promotes clear communication among a care team and treating clinicians by ensuring awareness regarding patient care plans.
  • Updates patient care practices (for example: policies, protocols, clinical pathways, order sets) related to the care of the palliative care practice.
  • Interacts with and contributes to the professional development of peers and colleagues; participates on a team for data collection, health care outcome reporting, clinical audits, and programmatic evaluation.
  • Collaborates with patient, family, and others in the conduct of nursing practice; acts as a liaison between in-patient and out-patient palliative services.
  • Integrates ethical provisions in all areas of practice; utilizes the ethic decision making model to drive issues and advocate for patient wishes.
  • Considers factors related to safety, effectiveness, cost and impact on practice in the planning and delivery of nursing services.
  • Provides leadership in the professional practice setting and the profession.
  • Practices within the scopes and standards of practice set forth by the American Nurses Association and other professional organizations specific to specialty.
  • Supports additional projects related to care coordination across the health system.
  • Performs other duties as assigned.

  • Basic computer skills and knowledge of managed care
  • Working knowledge of insurance coverage and procedures and community resources
  • Strong organizational, interpersonal, written, verbal, teamwork, critical thinking skills
