Job Objective:

Plans, organizes, coordinates, and executes all functions and activities associated with the 24/7 Recognition Program, requiring open, frequent, and responsive communication to meet 24/7 donor needs. Acts as the key point of contact within the healthcare system for program donors, ensuring timely access to a variety of healthcare programs and services in gratitude for their contributions.

Job Description:

Education:

Required: Bachelor’s degree in Nursing or Master’s degree in Nursing required if hired into position after October 1, 2023

Licensure/Certification:

Required: California Registered Nurse (RN) License; American Heart Association BLS

Experience:

Required: Five (5) years of recent patient care experience Preferred: Cumulative patient care experience in a variety of care delivery settings (acute, ambulatory, case management, etc)

Essential Responsibilities:

  • Intervenes appropriately to reduce anxiety and enhance patients' ability to follow through with appointments and prescribed care.
  • Communicates and bridges knowledge gaps, utilizing teachable moments to provide up-to-date medical information on diagnostic testing and planned treatments; wisely recognizes when a member’s need for information or support is beyond personal knowledge and facilitates timely contact with the appropriate entity.

Monitors treatment activities, tests, and medication orders while advocating to support timeliness. May assist, in close collaboration and with permission from the primary assigned ED nurse, with the delivery of general nursing care to improve the patient experience (excluding assessments and medications). Takes action to begin work on referrals or prescribed follow-up measures upon discharge.
  • Collaborates with in-patient staff to facilitate a smooth hospital admission and stays with the patient throughout the admission process as appropriate. Assures that the patient will follow up on their care daily.
  • Conveys through actions and communication an appreciation for the central role
  • Shares knowledge and acts as a professional resource to colleagues and other members of the healthcare team. Trains, orients, coaches, and positively supports new hires to ensure confident, seamless integration into the department, boosting new team members' ability to uphold safety, care quality, and outstanding customer service.
