  • Job Objective: A brief overview of the position.
    • Provides direct patient care and support to multidisciplinary health team working within the scope of practice of a Family Nurse Practitioner (FNP). The FNP will serve as clinical area expert to include patient care, education, collaboration, and consultation activities. Assumes responsibility for professional development and program maintenance. Contributes to the department budget by cost effective use of time and supplies. Demonstrates commitment to system values through customer focus and continuous process improvement.

  • Qualifications
    • Education
      • Required: Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)

    • Licensure/Certification
      • Required: Active and unrestricted California licensure as a Registered Nurse and Nurse Practitioner; Board Certified through ANCC or AANP; California Nurse Practitioner Furnishing number; DEA provider number schedule II-V; CURES registration; NPI Number; American Heart Association BLS

    • Experience
      • Preferred: 1 to 2 years experience in the area of specialty
  • Essential Responsibilities
    • Demonstrates compliance with Code of Conduct and compliance policies, and takes action to resolve compliance questions or concerns and report suspected violations.
    • Monitors department specific standards of care/practice.
    • Admits patients in collaboration with Site Director.
    • Develops plan of care based on assessment.
    • Demonstrates implementation skills in accordance with EMH Standards of Care, policies, and procedures
    • Demonstrates ability to change intervention strategies when expected outcomes not met.
    • Consistently documents in accordance with EMH policies and procedures.
    • Determines need for laboratory studies and orders and evaluates results.
    • Performs certain diagnostic and therapeutic procedures and develop and implement therapeutic plans to achieve nursing and medical goals. Communicates findings to attending physician.
    • Demonstrates competency in the operation of equipment specific to clinical area.
    • Provides accurate and precise reporting of patient condition and changes in a timely manner to physicians.
    • Serves as consultant or resource person in area of expertise for patients/families, nurses and other healthcare professionals.
    • Formulates, implements, and documents an individual teaching plan for each patient.
    • Consults other healthcare providers and coordinates medical care.
    • Provides psychosocial support to patients and family members.
    • Initiates discharge planning and teaching upon admission and in conjunction with Case Manager where appropriate.
    • Participates in continuous quality improvement and quality assurance activities and various clinical committees, as appropriate. adheres to guidelines for change of shift report and internal transfer of patient care.
    • Communicates with co-workers prior to breaks and lunches to ensure continuity of patient care.
    • Complies with departmental plan to meet budget goals.
    • Demonstrates knowledge of Department of Nursing performance and improvement and patient safety goals.
    • Takes call on rotational basis, if applicable.

    • Performs other duties as assigned.
