Job Objective:

Utilizes specific clinical knowledge to offer individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers. Using the nursing process provides education and resources to facilitate informed decision making and timely access to quality health and psycho social care throughout all phases of the care continuum. Works with patients referred for assistance in accessing and utilizing health care resources due to their complex chronic illness.

Education:

Required: Bachelor of Science in Nursing (BSN) or Master’s degree in Nursing

Licensure/Certification:

Required: California Registered Nurse (RN) licensure; Basic Life Support (BLS) from American Heart Association (AHA); Additional certifications required as outlined based on assigned department: Advanced Cardiovascular Life Support (ACLS) from AHA: Upon hire: CC7581 EDCC Cardiology Clinic Neonatal Resuscitation Provider (NRP) certification from AHA and American Academy of Pediatrics (AAP): Upon hire: 6400 2N LDRP 6530 Neonatal ICU Level II
Preferred: Certified in specialty practice area

Experience:

Required: Three (3) years of RN experience
Preferred: Experience in chronic disease management, case management, utilization management, adult acute care or area of specialty

Essential Responsibilities:

  • Demonstrates compliance with Code of Conduct and compliance policies, and takes action to resolve compliance questions or concerns and report suspected violations.
  • Assessment: Collect comprehensive data pertinent to the patient’s health or situation.
  • Diagnosis: Analyze the assessment data to determine the diagnoses or issues.
  • Outcomes Identification: Identify expected outcomes for a plan individualized to the patient or the situation.
  • Planning: Develop a plan of care that prescribes strategies and alternatives to attain expected outcomes across the care continuum for inpatient and/or outpatient procedures.
  • Implementation: Implement the identified plan.
  • Evaluation: Evaluate progress toward attainment of outcomes and revise as needed to meet goals.
  • Quality of Practice: Systematically enhance the quality and effectiveness of nursing practice using the Relationship Based Care model and shared governance for decision making.
  • Education: Attain knowledge and competency that reflects current evidence based practice for nursing care. Facilitates health and disease patient education, including leading group office visits. Supports patient self-management of disease and behavior modification interventions.
  • Professional Practice Evaluation: Evaluate personal nursing practice in relation to professional practice standards and guidelines, relevant statutes, rules and regulations. Promotes clear communication among a care team and treating clinicians by ensuring awareness regarding patient care plans.
  • Collegiality: Interact with and contributes to the professional development of peers and colleagues. Participates on a team for data collection, health care outcome reporting, clinical audits, and programmatic evaluation.
  • Collaboration: Collaborate with patient, family, and others in the conduct of nursing practice.
  • Ethics: Integrate ethical provisions in all areas of practice. Utilizes the ethic decision making model to drive issues and advocate for patient wishes.
  • Research: Integrate research findings into practice. Monitor the progress of patient through the service plan and re-evaluate the effectiveness and appropriateness of the plan based on the patient’s condition and needs.
  • Resource Utilization: Consider factors related to safety, effectiveness, cost and impact on practice in the planning and delivery of nursing services. Provide patients linkages to services to promote a pathway to care and positive outcomes.
  • Leadership: Provide leadership in the professional practice setting and the profession.
  • Practices within the scopes and standards of practice set forth by the American Nurses Association.
  • Practices within the scopes and standards set forth by professional organizations specific to specialty.
  • Participates in the development and ongoing success of certification and accreditation for the assigned program/department, as appropriate.
  • Supports additional projects related to care coordination across the health system.
  • Performs other duties as assigned.
