  • Job Objective: A brief overview of the position.
      • Under the direction of Eisenhower Concierge Nursing (ECN) executive leadership and in close collaboration with department Registered Nurses and Licensed Vocational Nurses, the Nursing Assistant performs nursing assistant skills within their scope of practice.

  • Qualifications
    • Education
      • •    Required: High School Diploma, GED or any higher level education degree if hired on/after April 10, 2016

    • Licensure/Certification
      • •    Required: Certified Nursing Assistant (CNA) if hired/promoted after April 10, 2016; American Heart Association BLS; CA Driver License and auto insurance
        •    In lieu of certification, must have completed one (1) semester of education in an accredited ADN or BSN program. 

    • Experience
      • •    Preferred: Nursing Assistant experience 

  • Essential Responsibilities
    • 1.    Demonstrates compliance with Code of Conduct and compliance policies, and takes action to resolve compliance questions or concerns and report suspected violations.
      2.    Demonstrates exceptional commitment to patient confidentiality and adheres to HIPPA.
      3.    Continuously treats all clients and stakeholders with kindness, empathy, dignity, and respect, even in the presence of adverse circumstances. 
      4.    Assists with and is accountable for adhering to safe, high-quality patient/client care, within scope and according to prescribed nursing care plan.
      5.    Maintains a professional image and a clean, orderly environment during the delivery of care.
      6.    Actively promotes a supportive setting that embraces the physical, emotional, and spiritual aspects of health and healing.  
      7.    Direct patient care:
      •    Presents to clients home or requested site of service on-time, donned in organization approved attire/uniform, and in full compliance with dress code standards. 
      •    Adheres to best practice infection control per company policy.
      •    Monitors patient physical and emotional condition. Reports any changes in status promptly to Lead RN, or other appropriate RN, and documents vitals or other pertinent information accurately.
      •    Performs hygiene per standard.
      •    Appropriately positions and ambulates clients per plan of care.
      •    Assists with set up/and or feedings of clients.
      •    May be required to accompany clients to ensure safety during transport to provider appointments, testing, or procedures where appropriate. 
      •    Accurately performs duties assigned by and under the supervision of the healthcare team; does not deviate without the involvement of the Registered Nurse.
      •    Adheres to guidelines for change of shift report from assigned staff member
      •    Completes documentation accurately as per policy.
      •    Maintains generally tidiness of clients care area, and may be required to perform light housekeeping or meal preparation.
      8.    Facilitates client’s participation in all decisions concerning care and appropriately advocates on their behalf.
      •    Prioritizes care, commensurate with client’s needs.
      •    Effectively communicates pertinent information, and employs use of SBAR to handover care. 
      •    Intervenes appropriately to reduce anxiety and enhance patients’ ability to follow through with prescribed care, and aspires to build/maintain patient trust.
      •    Assists other staff in the delivery of care, as appropriate
      9.    Recognizes potentially critical situations and takes action to minimize adverse outcomes.
      10.    Actively seeks opportunities to support/up-lift department morale.
      11.    Earnestly promotes a positive organization image when interfacing with the public/clients, medical system staff/professional, and either personally takes action to address issues and/or immediately informs and engages ECN executive leadership.
      12.    Ensures appropriate capture of charges for supplies utilized in the course of care.
      13.    Takes call for the department on a rotational basis.
      14.    Maintains reliable transportation and utilizes personal vehicle to transport oneself to site of contracted client; accurately accounts for miles traveled (mileage reimbursed at current IRS defined rate per mile).
      15.    Maintains commitment to professional development and clinical competency.
      •    Attends requested professional development sessions and staff meetings. If unable to attend meetings, reviews minutes or discusses proceedings with peers or manager. 
      •    Identifies personal knowledge deficits and seeks resources/consultation to ensure safe patient care.
      •    Strives to be a life-long learner, open to and utilizes constructive feedback; strives to complete a minimum of 12 hours of continuous education annually. 
      •    Embraces change and continuously identifies opportunities for improvement by demonstrating a commitment to utilizing evidence based practice.
      16.    Shares knowledge and acts as a professional resource to other members of the health care team
      •    May be asked to train, orient, and positively support new hires to ensure confident, seamless integration into the organization
      •    Boosts new team members ability to uphold safety, care quality, and outstanding customer service.
      17.    Performs other duties as assigned.
