Education:
Required: Bachelor’s degree in Nursing or Master’s degree in Nursing
Licensure/Certification:
Required: California Registered Nurse (RN) licensure; Basic Life Support (BLS) from American Heart Association (AHA); California Driver License and auto insurance
Preferred: Certification in one area of specialty
Experience:
Required: Five (5) years of RN experience which includes direct patient care in acute, ambulatory or home health
Preferred: Nursing leadership and teaching/mentoring experience
Job Objective:
Under the direction and oversight of Eisenhower Concierge Services (ECS) executive leadership assumes responsibility for organizing, coordinating and delivering exceptional quality, concierge nursing services within their assigned team.
Essential Responsibilities:
- Demonstrates compliance with Code of Conduct and compliance policies, and takes action to resolve compliance questions or concerns and report suspected violations.
- Compiles with all applicable ECS policies and procedures, including Code of Conduct, Compliance policies, and Relationship-Based Care Professional Practice Model; takes action to involve ECS leadership to resolve compliance questions or concerns and/or reports suspected violations.
- Recognizes potentially critical situations and takes action to minimize risk of adverse outcomes.
- Develops and maintains open, honest and mutually beneficial relationships with ECS leadership, staff, clients, and relevant stakeholders.
- Collaborates with ECS leadership and peers to continuously improve and streamline workflow processes to ensure and uphold high-caliber service and valuable resources.
- Follows policies, procedures, directives and works with executive leadership to update/improve when/where appropriate.
- Assists with and is accountable for the coordination of safe, high-quality patient/client care by properly triaging client care requests and determining the appropriate delegation of assignments to qualified caregivers.
- Coordinates caregiver schedules for the provision of client care.
- Directs patient care, as prescribed by ECS policy and procedure.
- Implements plan of care according to identified physical, psychosocial, and spiritual needs of patient.
- Ensures physician orders to initiate treatments and scheduling are available and viable; judiciously intervenes/connects with physicians to rectify issues.
- Regularly rounds with ECS leadership and staff in the field to assess and evaluate care and services provided.
- Recognizes, accepts responsibility for, and maintains commitment to professional development and clinical competency.
- Shares knowledge and acts as a professional resource to other members of the health care team, especially in relation to new hire onboarding.
- Trains, orients, and positively supports new hires to ensure confident, seamless integration into ECS and boosts new team members’ ability to uphold safety, care quality, and outstanding customer service.
- Monitors and audits electronic communication and electronic charting system for compliance and adherence to company policy/best practice.
- Facilitates availability of adequate material resources and supplies, and ensures appropriate capture of charges for supplies utilized in the course of care.
- Monitors correct timekeeping of staff through routine/daily review of timecards, checking to ensure shifts are in line with posted schedules where appropriate.
- Ensures personal work availability is adequately flexible and is in concert with other Care Coordinators, to allow for necessary coverage to meet operational need.
- Embraces change and continuously demonstrates a commitment to utilize evidence-based practice.
- Seeks opportunities to support/uplift ECS morale.
- Earnestly promotes a positive organization image when interfacing with the public/clients, medical system staff/professionals, and either personally takes action to address issues and/or immediately informs and engages ECS executive leadership.
- Maintains reliable transportation and must be willing to utilize personal vehicle to transport oneself to site of contracted client; engage in honest, accurate accounting of miles traveled (mileage reimbursed at current IRS defined rate per mile).
