    • This position will support the daily functions of the Hospitalist Group Department and utilize all channels of communication to obtain knowledge, transmit information, address concerns and solve problems.

  • Qualifications
    • Education
      • Required: BSN 
    • Licensure/Certification
      • Required: California RN license
    • Experience
      • Required: Minimum 3 years as a RN
      • Preferred: One year Nursing Inpatient
      • Preferred: Experience in performance / quality improvement activities
  • Essential Responsibilities
    • Demonstrates compliance with Code of Conduct and compliance policies, and takes action to resolve compliance questions or concerns and report suspected violations.
    • Interprets laboratory and test results and communicates abnormal findings to attending physician as needed.
    • Abstracts and prepares data for quality review, including clinical indicators, patient satisfaction indicators, and standards of care indicators.
    • Assists physicians with daily routine tasks including medication authorizations and completion of forms.
    • Schedules patient appointments with PCPs upon discharge from the hospital.
    • Performs post-discharge patient call backs to gage patient experience, address inquiries and remind patients of follow up appointments.
    • Responds to telephone inquiries from patients regarding clinical issues.
    • Communicates with hospital staff, hospital personnel and hospital practitioners to maintain continuity of patient care, improve performance and resolve issues.
    • Counsels patients on health care options.
    • Works in collaboration with a multidisciplinary healthcare team to provide and direct patient care and support.
    • Is a good steward of EMC/Hospitalist’s resources by cost effective use of time and supplies.
    • Performs other duties as assigned.
